Nurse Practitioner (Registered Nurse Extended Class) Primary Care The Kingston Family Health Team (KFHT) has an opening for a temporary full time Nurse Practitioner position covering a medical leave.

General Overview:

The Nurse Practitioner (NP) works in close collaboration with physicians and Interdisciplinary Healthcare Providers (IHPs) within the Kingston Family Health Team. As a vital member of the patient focused team, he/she provides clinical services in conjunction with all health care professionals. As an advanced practice nurse,



the NP will function within his/her full scope of nursing practice.

Summary of Duties:

· Providing accessible, comprehensive and effective care to patients of all ages, with a focus on preventative care and health education.

Qualifications include:

· Current unrestricted extended class certificate and registration with the College of

Nurses of Ontario as a Nurse Practitioner-Primary Health Care (NP-PHC).

· Graduate of a recognized Primary Health Care Nurse Practitioner program.

· Primary care experience preferred.

· Proficiency in Microsoft Office and Electronic Medical Records

· Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team.

· Proven analytical and problem-solving skills.

· Excellent communication and interpersonal skills.

· Demonstrated ability to multi-task.
