About the IPCT Partnership

This is a partnership position between Women's Health in Women's Hands Community Health Centre (WHIWH CHC) and Magenta Health, funded through the Ministry of Health's Interprofessional Primary Care Team (IPCT) Initiative.

The successful candidate will be an employee of WHIWH CHC and will work as part of the Magenta Health clinical team. The role is governed by WHIWH CHC's terms and conditions of employment, while day-to-day clinical practice will occur within Magenta Health's primary care environment.

The Nurse

Practitioner will work collaboratively with physicians, nurses, administrative staff, and other healthcare professionals to improve patient attachment, access to care, and the delivery of comprehensive primary care services. The successful candidate may be required to provide care across Magenta Health's Riverside, Beaches, and Midtown clinic locations, as well as through virtual care modalities, to support patients attached to participating physicians across the Downtown East Ontario Health Team (DETOHT) catchment area.

RESPONSIBILITIES

Patient Attachment and Intake (Primary Focus)

- Conduct comprehensive intake assessments for newly attached patients.
- Complete health histories, physical examinations, screening, diagnostic evaluation, and treatment planning.
- Establish initial care plans and facilitate ongoing attachment to primary care services.
- Support patients in navigating available clinical services and referral pathways.




- Collaborate with physicians and clinic staff to improve patient access and continuity of care.

Urgent and Episodic Care
- Provide urgent and episodic primary care services for patients presenting with acute health concerns.
- Perform clinical assessments, establish diagnoses, order and interpret diagnostic tests, prescribe medications, and develop treatment plans within the full scope of Nurse Practitioner practice.
- Assess, diagnose, and manage acute and chronic conditions in accordance with College of Nurses of Ontario standards and evidence-informed practice.
- Collaborate with physicians and interdisciplinary team members to ensure timely and coordinated care.
- Provide urgent and episodic primary care services, including occasional evening (5:00 p.m. to 8:00 p.m.) and weekend (9:00 a.m. to 12:00 p.m.) coverage to support timely patient access to care.

Comprehensive Primary Care and Program Development
- Provide comprehensive primary healthcare across the lifespan.
- Support prevention, health promotion, disease screening, and chronic disease management initiatives.
- Participate in the development and implementation of specialized clinical services, including sexual health, HPV/STI clinics, PrEP services, diabetes management, menopause care, and other priority programs.
- Contribute to the planning, implementation, monitoring, and evaluation of treatment, education, counselling, and health promotion programs and services for individuals and groups.
- Participate in activities related to the development, implementation, and evaluation of medical directives, clinical protocols, policies, procedures, and best practices.
- Provide care through a combination of in-person and virtual modalities, as appropriate, to support accessible and patient-centred service delivery.

QUALIFICATIONS:
1. Undergraduate degree in nursing from a recognized university
2. Registration in the Extended Class with the College of Nurses of Ontario
3. Hold a Prescribing Narcotics and Controlled Substances certificate endorsed by the CNO.
4. Three to five years of nursing experience in a community setting or a combination of community and hospital or public health settings; experience in a primary care setting, an asset.
5. Demonstrated critical thinking and problem-solving skills,



organizational and interpersonal skills with an ability to work effectively and respectfully in a collaborative, inter-professional team workplace.
6. Experience in program development, implementation, monitoring and evaluation.
7. Knowledge of women’s health issues.
8. Experience in prenatal care is an asset
9. Proficiency in the use of computers and various software applications.
10. In addition to English, knowledge of a language spoken within one of the four priority communities, an asset.

REPORTING The Nurse Practitioner will be employed by Women’s Health in Women’s Hands Community Health Centre and deployed to Magenta Health through the Interprofessional Primary Care Team (IPCT) Initiative. Administrative supervision will be provided through WHIWH CHC, while day-to-day operational supervision, clinical integration, and collaboration will occur within the Magenta Health practice environment.
