Villa St. Gabriel Villa is a 128-bed long-term care home providing the highest standards of resident care. The home is located in Chelmsford. Our approach to holistic care respects the unique needs and fosters the spiritual, physical and emotional well-being of each of our residents.

Working with the residents onsite at Villa St. Gabriel Villa the Nurse Practitioner plans, provides, supervises, and ensures direct care for residents in accordance with the plan of care and established policies and procedures. The Nurse Practitioner is responsible for being part of the dayto-day operation of the medical services team as well as the largerinterprofessional team.

Additional responsibilities include but are not limited to:

- Perform Comprehensive Geriatric Assessments (CGA) and geriatric consultations for residents with complex and challenging health care needs across programs.
- Work in partnership with leadership and direct patient care staff to develop, implement and evaluate best practices in geriatrics care across the organization.
- Provide medical liaison with local hospital(s) and other appropriate health care organizations or facilities.
- Cooperate with, and/or be available during,



Ministry compliance reviews pertaining to medical issues.
- Appropriate assessment and management of controlled substance prescribing within the geriatric context.
- Participate in the identification of clinical or operational performance improvement opportunities and assist in performance improvement activities.

QUALIFICATIONS:

- Current Nurse Practitioner Registration with the College of Nurses of Ontario in the Extended Class
- Preference for experience in the care of geriatric clients and adult chronic disease management.
- Current CPR certification
- French Language Service Designation: Bilingualism, English and French, is required. In accordance with its designation under the French Language Services Act, if no bilingual candidates (both internal and external) meet the qualifications of the position, unilingual English-speaking candidates may be considered.
