Your role will involve close collaboration with various healthcare professionals. Key Responsibilities:

- Complete initial assessments within 24 hours of referral
- Conduct follow-up visits to facilitate client progress
- Align care with standards of HCCSS/OH@H




- Create and implement individualized therapy plans
- Educate clients and families on maintaining a protected home environment Requirements:
- Relevant experience in home health care preferred
- Current registration with the College of Occupational Therapists of Ontario
- Expertise in functional assessments and safety evaluations
- Ability to manage documentation efficiently
