THUNDER BAY REGIONAL HEALTH SCIENCES CENTRE is seeking a Patient Flow Coordinator, Hospital to Home (H2H), to optimize day-to-day patient flow and coordinate home care services in Northwestern Ontario. The role emphasizes PFCC, discharge planning, and interprofessional collaboration.

The incumbent will work under the H2H manager and nursing leadership, ensuring timely referrals, robust communication, and adherence to CNO standards. A valid Ontario RN license and solid teamwork are essential.
