This role will focus on delivering person-centered nursing in a collaborative environment, requiring solid assessment skills and the ability to educate patients effectively. You will engage in direct patient care while ensuring quality health management and rehabilitation. Key Responsibilities:

- Perform patient assessments and develop care plans
- Coordinate care delivery within the health care team
- Provide education to patients and families on health issues
- Maintain accurate medical documentation
- Participate in nursing committees and initiatives Requirements:
- Graduation from a nursing program approved by the Canadian Nurses' Association
- Registration with BCCNM
- Minimum two years in cardiac prevention and rehabilitation
- Valid BCLS and ACLS certifications
