Clients specific post - Looking for a experienced nurse to assist in home with 2 trach and vent patients - Schedule needs: must be available 2200-0800 or 10am-6pm, every other weekends also a must.

Pls do not apply if you do not have experience or the availability

Key Responsibilities

- Tracheostomy care and management
- > Vent 24/7, passive circuit

- -wet circuit in bedroom and dry on wheelchair
- > trach dressing changes daily and PRN

- -Vashe wash, vashe soaked gauze compress x5 mins, AMD gauze
- Suctioning and airway management
- -inline suction catheter
- Enteral feeding and PEG tube care and management
- -kangaroo pump
- -end feed, wash bag and hang to dry
- -High risk for UTI. Ask mom you think extra hydration is needed
- Wound care and skin integrity management
- -wounds on buttock and back almost healed
- > Apply mepilex as needed for any redness.

- -monitor skin
- Appropriate positioning to facilitate ventilator.
- -wears ear plugs in his nose to prevent the leak

Experience & Qualifications

- Minimum of 1 year of experience as a Registered Practical Nurse (RPN).
- Experience in acute care, dementia care, hospice care, or other specialized areas is an asset.
- Robust skills in medication administration and familiarity with various administration routes.
- Knowledge of pain management protocols and best practices.
- Familiarity with medical office procedures and healthcare documentation.
- Demonstrated ability to provide compassionate, patient-centered care.
- Exceptional communication skills for effective interaction with patients, families, and healthcare teams.

Requirements

- Current registration with the College of Nurses of Ontario (CNO).
- Completion of an RN certificate program.

Pay: $32.00-$35.00 per hour

Application question(s)

- Are you comfortable providing care for a client with a cuffless tracheostomy, including trach-site assessment, suctioning/airway care, cough-assist, ventilator checks, and recognizing signs of respiratory distress?
- Do you have hands-on experience with G-tube feeding and medication administration, including setting up overnight feeds, flushing the tube, monitoring tolerance, and documenting intake/output?
- Are you experienced with total-assist transfers using a ceiling lift and power wheelchair, including safe two-person transfers and proper positioning/repositioning of a non-weight-bearing client with very limited range of motion?
- Are you comfortable caring for clients with complex skin-integrity needs, including pressure-ulcer prevention, frequent skin assessment, positioning, use of specialized cushions/orthotics, and reporting changes promptly?
- This position involves caring for two highly dependent clients in the same home, detailed respiratory and clinical monitoring, following established family care routines, and documenting all care thoroughly. Can you independently follow a detailed care plan while recognizing and promptly escalating changes in the client's condition?
- Do you live in Hamilton?
- Do you have hands-on nursing experience caring for a client who requires long-term mechanical ventilation (such as an Astral ventilator) and a tracheostomy, including respiratory monitoring and airway management?
- Can you work Overnights?

Licence/Certification:
- Driving Licence (preferred)
- Registered Nurse license (required)
