Key Responsibilities:
• Assess and adjust individualized care plans as needed
• Conduct educational sessions for patients and families




• Prepare detailed reports on patient care activities
• Mentor and support new healthcare team members
• Participate in research and quality improvement initiatives

Requirements:
• Graduation from an approved nursing program
• Two years relevant experience with diabetes patients
• Certified Diabetes Educator credential desirable
• Active RN registration with BCCNM
• Current CPR certification
