Summary Supports the mission of the hospital by aiming to reduce hospital readmissions through patient discharge optimization and follow-up. Performs clinical duties and engages in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourages provider follow-up and continuity of care following hospital discharge.
Minimum Qualifications Experience is PGY‑1 residency; PGY‑2 residency.
Education Bachelors Degree
Doctoral Degree
Pharmacy specialization
Education Equivalent Experience Bachelors – Pharmacy (required)
Doctorate – Pharmacy (preferred)
Credentials Pharmacist (essential)
Pharmacist – State of Florida (equivalent required)
ASHP (Pharmacists) (preferred)
Facility BayCare Health System, Specialty Pharmacy Clinic‑SJH
Location St Joseph's Main and Children's
Status PRN, Exempt: No
Shift Hours 8AM-4:30PM
Shift Shift 1
Weekend Work None
On Call No
