Detailed Overview
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and Fraser Health’s Mission, Vision and Values, the Client Care Coordinator supervises and provides work direction to staff, coordinates staffing coverage, offers clinical leadership and guidance, and acts as a clinical resource to the interprofessional team.

Responsibilities

Provides leadership to the interprofessional team by coordinating client care activities, offering information, and consulting designated care providers through methods such as reviewing client and family needs, troubleshooting, and providing guidance on client management.

Supervises and directs staff through scheduling, coordinating and adjusting work activities, establishing priorities, and providing performance feedback.

Determines training and orientation requirements, supports staff development, and contributes to new staff selection and performance evaluation.

Observes staff and students, reviews documentation, and investigates incident reports to provide interventions as needed.

Participates in the development and maintenance of safety programs, identifies safety concerns, and recommends corrective actions.

Works on quality improvement and risk management,



evaluating the impact of clinical practice changes and recommending alternative approaches.

Supports staff development through needs assessments, new skill introductions, and orientation of new staff.

Reviews client records for legibility and accuracy and recommends program effectiveness improvements.

Consults with interprofessional team members to resolve client and family issues, ensuring confidential communication and proper documentation.

Participates in committees and operational task groups, liaises with other programs, agencies, and hospitals.

Promotes research-based practice through collaboration and staff involvement in research activities.

Assists the Manager in planning, developing, and evaluating client care, including policy and procedure development.

Provides clinical leadership and consultation, ensuring compliance with standards and evaluating client needs.

Assists the Manager with administrative activities, resource utilization, budget input, and workload decision making.

Provides consultation and screening of clients in special circumstances and manages emergency client care issues.

Sets measurable personal performance goals and participates in professional development planning.

Assists staff with client care activities when required.

May oversee building maintenance requests and develop health and safety plans for licensed facilities.

Ensures licensing compliance and responds to inquiries and inspections for licensed facilities.

Performs other related duties as assigned.





Qualifications
Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three years of recent related clinical experience, including at least one year of supervisory experience, or an equivalent combination of education, training and experience. Completion of the required substance use certificate program. For licensed facilities: St. John’s Ambulance Safety-Oriented First Aid Certificate and current CPR level C or equivalent. For Mobile Integrated Crisis Response only: Enhanced Security Police Clearance.

Skills and Abilities

Capability to coordinate client assignments collaboratively with staff.

Skill in monitoring staff workload and adjusting client assignments accordingly.

Ability to set priorities for client care, patient flow, and overall daily operations.

Monitoring quality of client care and maintaining nursing practice standards.

Assessing and addressing performance issues and providing input on formal reviews.

Modeling, coaching, and mentoring best patient care practices.

Identifying learning priorities and supporting education based on changing practices.

Providing input into budgets and capital or minor equipment decisions.

Effective verbal and written communication skills.

Ability to work independently while remaining organized and self‑directed.

Problem‑solving and conflict resolution skills.

Operational knowledge of related equipment and software.

Physical ability to perform the duties required for the position.
