Key Responsibilities:
• Assess, plan, and evaluate patient care interventions
• Foster collaborative relationships with patients and their caregivers
• Lead the care planning and discharge process effectively




• Participate in quality improvement initiatives for patient care
• Document all patient interactions as per Fraser Health standards

Requirements:
• Current registered nurse certification with BCCNM
• Graduation from an approved School of Nursing
• Certified in Basic Cardiac Life Support
• Ability to advocate for patient needs sensitively
• Experience in a multidisciplinary healthcare environment
