As a member of the health care team, the Social Worker provides direct patient care through the provision of psychosocial screening and assessments; individual and family counseling; adjustment/coping; patient education; liaison and referral to community agencies; advocacy and discharge planning. Services are provided through individual, family and group counseling and by providing psychosocial expertise to the interdisciplinary team.
The Social
Worker participates within an interdisciplinary team setting in education, research and the development of the program.
- Masters of Social Work or equivalent
- Registered with, and in positive standing with the College of Social Workers and Social Service Workers of Ontario.
- Minimum 2 years experience in a health related setting
- Expert knowledge of community resources

Experience in a geriatric setting preferred
- Demonstrated clinical judgment and ability to utilize a holistic approach emphasizing quality of life
- Excellent clinical, interpersonal, communication and organizational skills in an interdisciplinary team setting
- Strong commitment to interdisciplinary care
- Demonstrated commitment to ongoing learning
