SW needed to advocate for clients to navigate the system and fill out applications and other paperwork to help them link of the resources they are eligible for.

Duties

- Conduct thorough assessments to understand clients’ needs, strengths, and challenges.
- Develop personalized care plans that align with clients’ goals and available resources.
- Provide counseling and emotional support to individuals experiencing crisis or ongoing difficulties.
- Connect clients with community services such as healthcare, housing, employment, and mental health programs.
- Advocate on behalf of clients to ensure they receive appropriate services and fair treatment.
- Maintain detailed case records, progress notes,



and documentation in compliance with organizational policies.
- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal professionals to coordinate comprehensive support.

Experience

- Proven experience in social work or related fields such as counseling, psychology, or community outreach.
- Robust understanding of social service systems, community resources, and client rights.
- Excellent communication skills with the ability to build trust and rapport quickly.
- Ability to handle sensitive situations with empathy, professionalism, and discretion.
- Familiarity with case management software and documentation standards.
- Knowledge of relevant laws, regulations, and ethical guidelines governing social work practice.
- Preferred certifications include Licensed Clinical Social Worker (LCSW) or equivalent credentials where applicable.
