Job Description Job Title: RN Case Manager
Are you an experienced RN with Case Management experience looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!
Position Summary:
As the RN Case Manager, you would be responsible for assessing, planning, implementing, coordinating, monitoring,



and evaluating case management activities across the continuum of care.
Hours for this Position: Monday-Friday (40 hours/week)

More Insight of Daily Responsibilities:
Check eligibility as per procedure/ client specifications
Assess initial clinical information and develop a treatment plan and goals that meet the individual patient's/ client's present needs
Communicate goals and objectives to all pertinent parties
Consult physician advisers to determine the medical necessity and appropriateness of treatment plan goals and objective
Determine the cost effectiveness and medical necessity of alternate treatment
Negotiate with providers when necessary to reduce patient cost
Qualifications What We Look For:
Clear and active RN license in the state of CA
3+ years of clinical practice experience
Case Management Certification preferred
