Registered Nurse - Primary Care (SIHN – Sooke, BC)
Provide direct patient care in Urgent Primary Care Centre, Patient Medical Homes, and primary care clinics. Deliver episodic and longitudinal care across all age groups while ensuring client‑ and family‑centered care with cultural humility and a trauma‑informed, evidence‑based approach.

Responsibilities

Conduct initial client assessment, health promotion, intervention, and education based on client needs and urgency.

Provide health coaching and teaching aligned with evidence‑based practice.

Facilitate and manage client transitions across the healthcare continuum to prevent avoidable hospital admissions and ED presentations.

Collaborate with the primary care team, community services, acute care, and other health professionals to create and implement care plans.

Operate within Island Health policies and procedures, including cultural safety standards.

Travel between sites as required, utilizing personal vehicle or Island Health transportation arrangements.

Qualifications

Registered RN with BC College of Nurses and Midwives.





At least two years of recent clinical nursing experience in primary or community care, including infants, children, youth, adults, and seniors.

Valid BC driver’s license.

Basic Life Support Level C.

Demonstrated ability to provide client‑ and family‑centered care with cultural humility using a trauma‑informed approach.

Knowledge of Indigenous peoples’ historical and current healthcare experiences.

Strong communication, documentation, interdisciplinary collaboration, and time‑management skills.

Physical ability to carry out the duties of the position.

Skills and Abilities

Ability to navigate scope of practice across disciplines.

Data integration and problem‑solving from multiple sources.

Health education in chronic disease and self‑management.

Effective verbal, written, and electronic communication.

Adaptability to dynamic environments and changing priorities.
