Join a dedicated team as an Occupational Therapist providing person-centred care to diverse populations in community and primary care settings. Focus on improving clients' daily activities and overall well-being. As a Community-Based Occupational Therapist, you will enhance mobility, safety, and quality of life for clients of all ages, including seniors and individuals with disabilities.
Collaborating within an interdisciplinary team, you will carry out assessments, develop individualized intervention plans, and provide education for clients and caregivers. The role emphasizes the importance of addressing both mental and physical health needs of clients in their settings. Key Responsibilities:
Conduct comprehensive, client-centred occupational therapy assessments
Develop and implement individualized, occupation-focused intervention plans
Provide assessments on home safety and fall risks
Deliver health promotion and disease prevention interventions
Support system navigation for clients and caregivers Requirements:
Master’s or Bachelor’s Degree in Occupational Therapy
Current registration with the College of Occupational Therapists of Ontario
Minimum three years in community-based settings
Experience with diverse populations, specifically seniors
