As a Field Case Manager in a 15-month contract, you will act as the primary contact for healthcare providers and patients. Your expertise will educate providers on program services and navigate reimbursement processes. With exceptional communication and organizational skills, you will truly impact patient experiences while adapting to their diverse needs.
Key Responsibilities:
• Serve as primary contact for physicians and clinics
• Educate providers on therapy, reimbursement, and patient resources




• Review patient charts and manage documentation submissions
• Maintain ongoing communication about patient status
• Adapt program elements for quality improvement
Requirements:
• Registered Nurse (RN) or equivalent degree
• Knowledge of reimbursement processes
• Previous case management experience preferred
• Robust time management and analytical skills
• Bilingual capability is an asset for Quebec
