Take on the role of Intensive Case Manager in the supportive SAMH Team in Guelph-Wellington. Engage with community members facing addiction and mental health challenges through targeted case management efforts. This full time, permanent position requires independent work in a variety of community settings.

You will provide housing assistance, coordinate care, and conduct assessments while balancing evening and weekend hours. Collaborating with community partners enhances your ability to create impactful support plans and facilitates client access to essential services. Key Responsibilities:
Facilitate intake and process client referrals
Assist with move-in logistics and access to entitlements
Construct and implement personalized care strategies
Conduct monthly home visits and service evaluations
Ensure thorough documentation and confidentiality Requirements:
Completion of a relevant Diploma or Degree
At least one year of experience in mental health/addiction
Familiarity with case management techniques
Current G Driver’s License and transportation
Certification in First Aid/CPR Make a difference in addiction support and mental health through dedicated case management in Guelph-Wellington.
