Public or community health centre Responsibilities

Collaborate with members of an interdisciplinary health team Coordinate patient care Evaluate patient care and progress Implement an interdisciplinary plan of care Plan patient care Advise patients on drug and alcohol rehabilitation programs Ensure infection control Conduct patient interviews, physical assessments and take medical histories Prescribe and administer medications and treatments Prepare a treatment plan for each patient Perform administrative tasks Coaching Assess patients to identify appropriate nursing interventions Address and document symptoms and changes in patients’ conditions Provide consultative services regarding issues relevant to nursing profession and nursing practice Provide nursing care Operate or monitor medical apparatus or equipment Provide nursing services, within defined scope of practice, to patients based on patient assessment and care planning procedures Monitor patients' progress, evaluate effectiveness of nursing interventions and consult with appropriate members of healthcare team Supervision
