Transform patient care as a Nurse Coordinator for the CTEPH Program at UHN's Toronto General Hospital. This part time role focuses on comprehensive support for patients with chronic thromboembolic pulmonary hypertension (CTEPH).
As a Nurse Coordinator, you will play an essential role in managing the diagnostic and treatment process for CTEPH patients. Collaborating with multidisciplinary teams, you will coordinate diagnostic tests, triage referrals, and ensure timely access to specialized care. Your expertise will drive clinical excellence and enhance patient engagement throughout their healthcare journey.
Key Responsibilities:
• Coordinate patient assessments within the CTEPH program
• Triage referrals and facilitate timely consultations
• Oversee diagnostic testing coordination
• Monitor patient progress during diagnosis and treatment
• Deliver education on CTEPH and management strategies
Requirements:
• Bachelor's degree in Nursing required
• Current CNO registration and B.C.L.S. certification
• Relevant clinical experience in specialized areas
• Strong organizational and communication skills
• Ability to work in a multidisciplinary team
