The Systems Navigator provides intake services for new clients, and provides a comprehensive assessment of their strengths, capacities and needs, based on the social determinants of health. They will assist clients in navigating the broader health care system while also establishing and maintaining strong relationships with community partners. The Systems Navigator also coordinates physician engagement and delivery of service to clients across a continuum of care, facilitating and ensuring the achievement of quality health outcomes.
The Compensation Package A starting salary range of $51,000 - $55,000

100% employer-paid health & dental package, including up to $2,000/benefit year for mental health, $1,500 for massage therapy
Healthcare of Ontario Pension Plan (HOOPP)
Paid sick and personal time, plus vacation time starting at 3 weeks
Professional development time and funds
Required Qualifications Postsecondary education in social services or related field
Minimum 3 years of experience in community, health and/or social services working with vulnerable populations
Ability to assess and evaluate clients’ health and social needs, as expressed by the client
Extensive knowledge of community-based services and resources
Knowledge and understanding of forms that need to be completed to assist clients across the system
Demonstrated ability to work with individuals from diverse backgrounds through an inclusive and anti-oppressive lens
Access to a reliable vehicle and comfortability traveling within the community




Preferred Skills Awareness and understanding of the social determinants of health and health promotion
Ability to practice from social justice, trauma and violence informed care, and harm reduction frameworks
Excellent communication and interpersonal skills
Robust computer skills, including the use of Electronic Medical Records (Telus Practice Solutions Suite currently in use)
French and other language skills are considered an asset in supporting the communities we serve
