Position Summary The Registered Nurse, working as the Medly CRM Clinical Coordinator, collaborates with the interdisciplinary team of the UHN CaRE (Cardiovascular, Renal, and Endocrine) Clinic to support the delivery of high-quality care for patients with cardio-renal-metabolic conditions. This role is embedded within a pilot quality improvement project adapting the Medly remote management platform for cardio-renal-metabolic care, while concurrently supporting day-to-day care delivery within the clinic. Responsibilities include monitoring and triaging daily Medly alerts, communicating with patients and families to support proper use of Medly, and providing education on disease management and self-care. The Medly Coordinator applies clinical knowledge and best practice guidelines to support guideline-directed medical therapy,
including medication titration, while ensuring accurate documentation within the EPIC health information system. In addition to clinical responsibilities, the nurse contributes to the pilot by onboarding and consenting patients, extracting clinical data, and supporting evaluation activities that inform ongoing program refinement. The Medly Coordinator prioritizes workload effectively, contributes to continuous quality improvement initiatives, and promotes a collaborative and patient-centered care environment.
Duties: Recruitment of pilot participants (e.g. identify and screen potential participants, obtain informed consent)
Manage the daily (Monday-Friday) Medlyalerts in partnership with the UHN CaRE Clinic team
Communicate with patients and families and reviewproper use of Medly; prioritizealerts and accelerate to MDs as appropriate
Provide patients and families with CRM education on topics such as signs and symptoms of adverse reactions and self-care management of CRM
Use Medly to support Guideline-Directed Medical Therapy for CRM and titration of medications
Provide clinical care for patients participating in the Medly CRM pilot project and in the UHN CaRE Clinic
Execution of all aspects of quality improvement project activities (e.g. assessment of adverse events, monitoring safety, clinical data extraction) as per UHN policy and ICH/GCP guidelines
Document actions and communication according to College of Nurses of Ontario standards using EPIC health information system
Bachelor of Science in Nursing; Masters preferred
Registered Nurse with a minimum of 3 to 5 years clinical and/or professional experience
Current registration with the College of Nurses of Ontario
Previous clinical research experience preferred
Knowledge and experience with relevant guidelines and titration of cardiac, renal, or endocrine patients an asset.
Strong organizational and time management skills
Attention to detail and meticulous documentation practice
Excellent interpersonal and customer service skills
Excellent computer skills including Microsoft Office and EPIC applications
Excellent verbal and written communication skills
Strong analytical and problem-solving skills
Demonstrated ability to cultivate and build relationships
