Description Phoenix Residential Society, a community based health care organization, has an opening for a full time position as Intensive Case Manager in our Housing and other Management and Engagement Services (HOMES)
The Intensive Case Manager position is an integral part of a team-based approach in the provision of support services, referrals, advocacy, and system navigation to those individuals who were chronically homeless. Using a person-centred approach, provide case management services for a caseload of up to 12 participants.
Qualifications University degree in human services field (Social Work, Human Justice, Psychiatric Nursing, Chemical Dependency)




Minimum of two years experience working in the field of homelessness, mental health, and addictions.
Must possess as strong working knowledge of the Housing First framework.
Must possess a strong knowledge and understand of systems, supports, and available community resources for the successful delivery of services for persons that are chronically or episodically homeless including income supports, life skills support, health, mental health, and addictions services.
Must possess a strong working knowledge of addictions models including stages of change and harm reduction
Ability to work independently and as a team
Must demonstrate a proactive, hands-on whatever it takes approach to the provision of supports and services
Must utilize a non-judgmental therapeutic trauma-informed care approach at all times.
Strong organizational skills and ability to prioritize and manage multiple tasks simultaneously.
Must possess valid Class 5 drivers license and have daily access to a reliable vehicle to transport residents and execute daily duties
Criminal record check required upon hire.
Must have valid First Aid/CPR certification.
