Support the vital mission of Parkdale Queen West Community Health Centre as a Case Manager focused on primary care and client advocacy. This role is centered in mid-west downtown Toronto.
This full time, temporary position addresses community health needs for individuals facing barriers. Working alongside an interdisciplinary team, you will provide case management while advocating for essential client services. Your work will involve community collaboration to enhance resource availability and policy effectiveness for vulnerable groups.
Key Responsibilities:
• Deliver comprehensive, client-focused case management
• Advocate for clients in access to housing and legal services
• Maintain and manage client records efficiently




• Build relationships with community agencies for shared goals
• Participate actively in Centre events and training
Requirements:
• Degree in relevant field required
• Minimum of 3 years in case management or service navigation
• Experience with crisis intervention practices
• Ability to work within multidisciplinary teams
• High proficiency in computer systems and software
