Join the South East Grey Community Health Centre as a Primary Care Nurse Practitioner in Dundalk. This full-time role (37.5 hours/week) offers a contract starting October 2026.

We're looking for a skilled Nurse Practitioner who will work collaboratively with a multidisciplinary team. In this dynamic healthcare environment, you'll engage with physicians, nurse practitioners, and other health professionals while making a difference in community health. Your adaptability and sound clinical judgment will be essential in providing inclusive care to diverse populations.

Key Responsibilities:
• Collaborate with interdisciplinary teams for optimal patient care
• Work independently with initiative and sound clinical judgment
• Adapt to changes in a energetic healthcare environment




• Foster inclusivity and professionalism with patients and colleagues
• Develop chronic disease management strategies and programs

Requirements:
• Current registration with the College of Nurses of Ontario
• Minimum of 3-5 years of primary care experience
• Strong computer skills, including electronic medical records
• Understanding of the Community Health Centre model
• Experience with diverse or underserved populations
