**Position Description**
As a **Patient Information Clerk**
on 8A, Inpatient Haematology/Oncology, you will frequently be the first point of contact when patients and families arrive to the department.

You will provide service in a customer-focused manner.

You will facilitate the delivery of patient care by providing a high level of support and assistance to patient cluster staff and patients/families.

You will assist with patient registration/admissions/discharges, ensure thorough communication, manage documents, equipment, and supplies, and provide well-rounded customer service.

You will also support the operation of the team/department by providing clerical and administrative functions in a proactive, organized manner.
**Here's What You'll Get To Do**
Greet patients and families and assist in meeting their needs when appropriate.

Responsible for patient booking, registration, admission processes.

Schedule and coordinate meetings and create admission encounters in our electronic health record, EPIC.

Answer the telephone and respond to requests for information on the unit.





Facilitate transfer of patients between patient care areas.

Schedule and coordinate tests, procedures, return/follow-up appointments as necessary.

Act as a liaison between patient care area and other departments/units across the hospital with regard to patient procedures.

Work closely with the members of the ambulatory program to ensure transitions are efficient and smooth for our patients.

Coordinate unit administrative functions to ensure efficient operation of the unit.

Identify improvement opportunities related to patient flow, administrative processes and technologies and take the initiative to problem-solve with others.

Prepare requisitions, referrals, labels, and other documents.

Organize and maintain patient charts, as well as electronic and paper information filing systems.

Maintain electronic calendar/schedules.

Coordinate unit administrative functions to ensure efficient operation of the unit, within scope of authority, such as ensuring all preventative maintenance is arranged and completed in the required timelines.

Perform other duties as assigned.

Be a member of a high functioning, highly skilled, dedicated team.

Work within a agile care environment and with a unique patient population.

Work in a clinical research environment.
**Here's What You'll Need**
**Essential Requirements**
Diploma from a recognized Medical Administration program or 2+ years related work experience.

Proficiency with computer applications, including Microsoft Office Suite (Outlook and Word).

Excellent communication, interpersonal, and organizational skills.

Demonstrated ability to be punctual, reliable, and adaptable.

Strong critical thinking and problem-solving skills.

Ability to be self-directed, flexible, and to take initiative.

Excellent attention to detail and efficient time management skills.

Customer service oriented.

Demonstrated commitment to equity, diversity and inclusion (EDI) and experience advancing EDI objective.
**Assets**
Knowledge of medical terminology.

Previous administrative experience in a hospital/medical environment.

Experience with patient registration systems,
