The M-HEART program will operate five days per week in partnership with Northumberland OPP, Port Hope Police Services and Cobourg Police Services.



The M-HEART Mental Health Nurse is an experienced Registered Mental Health Nurse who works in partnership with Police as well as being an active member of the multi-disciplinary team at Northumberland Hills Hospital Community Mental Health Services. The M-HEART RN will represent Northumberland Hills Hospital and the Community Mental Health Program when performing his/her duties in the community. The M-HEART RN reports directly to the Clinical Manager and is positioned in one of the partnered police services in the M-HEART program. This position will be stationed at the Campbellford OPP detachment. The days and hours will be based on the operational needs of the program and community.Primary ResponsibilitiesThe M-HEART Nurse provides community mental health nursing care to individuals in mental health crisis and is deployed by police personnel or dispatch to perform crisis assessments, short-term follow up and support to individuals in crisis who may be experiencing mental health issues.Initial contact with clients will be in tandem with police officers attending calls for service. The M-HEART Nurse will provide consultation and crisis intervention with police to determine a therapeutic course of action for crisis intervention and response.The M-HEART RN will provide follow-up consultation, support and system navigation to individuals, families, caregivers, community professionals and partners on a short-term basis, to promote stabilization, service delivery access and collaboration.The M-HEART RN will perform mental status assessments, crisis response intervention, psychosocial assessment of the client's needs, medication assessment, symptom management, system advocacy, coordinate with other services including primary care, social services and/or other supports as appropriate. Community mental health nursing care will be provided to clients in a community-based setting; this may include but not be limited to completion of health assessments, medication monitoring in the community and providing injections in the community.The M-HEART RN will employ a client-centered and trauma-informed approach, utilizing recovery models to service delivery with a well-developed understanding of mental illness and concurrent disorder. Focused attention is given to the development of a therapeutic rapport and respect for the individual client.Assistance with connection to community resources for basic needs, emergency services, crisis services and other services as appropriate for the individual client and/or family.The M-HEART RN will work collaboratively with the client to develop a treatment plan that will include the client directed goals and identify appropriate care providers to assist with goals.The M-HEART RN will attend joint training events,



team meetings and participate in facilitating community workshops/seminars as per request.All documentation will be accurate and comply with NHH Community Mental Health Services Documentation Policy as well as the Ontario College of Nurses documentation requirements. Documentation will be completed in a timely manner and will include but not be limited to: client contact notes, initial treatment plans and crisis assessments.The M-HEART RN will be provided with electronic capability to complete documentation in a mobile setting and will maintain the equipment in good order with attention to electronic security.The M-HEART RN will complete the OCAN as well as the CCP and other documentation as required by the program, manager and/or police service partners.Qualifications/RequirementsCurrent certificate of registration and in valuable standing, with the College of Nurses of Ontario as a Registered NurseCNA Certification in psychiatric/mental health nursing, preferredCurrent and demonstrated knowledge of psychotic illnesses including etiology, prognosis and treatment modalities,Mental Health crisis assessment experience requiredMinimum 5 years' experience working with individuals with severe and persistent mental illness.Experience in mental health teaching to clients, family and communityRecent related clinical experience working with individuals with a serious mental illness preferably in a community settingWorking knowledge of Mental Health Reform, Mental Health Act, privacy legislationValid driver's license and insurance as incumbent will be required to drive their vehicle for the provision of mental health services.Successful completion of a criminal reference reviewCPR and First Aid Certification requiredPHYSICAL EFFORT AND WORKING CONDITIONSThe M-HEART RN will have the ability to work in a rapidly changing environment and to communicate clearly both verbally and in written formWork will be done in compliance with the Occupational Health and Safety Act; the M-HEART RN will actively participate in workplace and program orientation, education and trainingAt all times, the M-HEART RN will use any equipment, protective devices and/or clothing required by the employer in accordance with Occupational Health & Safety StandardsHours of work may be flexible and include some occasional evenings/weekends. The program operates five days per week.Employment RequirementsSuccessful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment.
