We value innovation, inclusivity, and continuous improvement.About the RoleWe are seeking a Primary Health Care Nurse Practitioner (PHCNP) for a permanent full-time position to provide holistic, client-centred care in a supportive and community-focused setting.Key Responsibilities- Conduct health assessments and physical examinations across the lifespan- Use evidence-informed clinical reasoning to formulate diagnoses- Collaborate with clients and care teams to establish and coordinate care plans- Identify trends in client health needs and contribute to program planning and advocacy- Maintain clear, accurate, and timely documentation (EMR, reports,
