You'll coordinate eligibility assessments, create supportive relationships, and assist clients in developing personalized recovery plans. Your work will connect them to essential resources like shelter, food, and health care. Key Responsibilities:
Review referral information of applicants to the program
Conduct eligibility screening interviews with applicants




Develop therapeutic relationships with assigned clients
Complete comprehensive assessments of client needs
Assist clients in creating personalized safety and recovery plans Requirements:
