Key Responsibilities

Provides comprehensive primary care to residents.

Is knowledgeable of the Resident Bill of Rights and endeavors to respect and promote it.

Conducts person‑centered health assessment/physical of residents on admission with specified time frame.

Complies with applicable provincial legislation pertaining to long‑term care, Fixing Long‑Term Care Homes Act.

Participates in regular focused resident and family care conferences.

Manages the care of residents by providing pharmacological, complementary and/or counseling interventions.

Provides outreach and transitional services to LTCH residents who return to the community.

Collaborates and consults with physicians, director of nursing and personal care nursing staff, interdisciplinary team and external resources regarding resident plan of care.





Provides leadership and involves the interdisciplinary team in quality improvement initiatives.

Provides formal and informal teaching and coaching in the management of clinical care.

Adheres to responsibilities and requirements for reporting adverse resident occurrences subject to regulatory and legislative direction.
