This position focuses on providing a range of client-centered services, emphasizing health promotion and disease prevention. Your wide-ranging responsibilities include patient assessments, immunization administration, and the development of chronic disease management programs. You'll also facilitate access to services for the community you serve.

Key Responsibilities:
• Assess patient needs and manage triage
• Track medical follow-ups for health screenings
• Conduct immunizations and allergy injections




• Perform clinical procedures and health assessments
• Participate in outreach nursing to augment service access

Requirements:
• Bachelor of Science in Nursing or equivalent
• Registered with the College of Nurses of Ontario
• 3+ years in community or public health nursing
• Experience in primary care settings is an asset
• Proficient in electronic medical records usage
