Key Responsibilities:
• Provide individualized support and wellness planning
• Assist clients with budgeting and financial management
• Facilitate access to necessary community resources




• Participate in advocacy initiatives for improved resource access
• Collaborate within a multi-disciplinary team environment

Requirements:
• Degree or diploma in health or social services
• Current registration with OCSWSSW
• 4+ years community-based experience with adults
• Preferred experience in congregate living settings
• Valid Canadian Full G driver’s license
