Description
This position is a permanent part‑time practice in long‑term care. Lakeview Care Centre, a 177‑bed facility, is currently served by three Long Term Care Initiative (LTCI) practitioners and requires an additional practitioner to assist with resident caseloads. In addition to LFP or fee‑for‑service payments, practitioners can join the LTCI, which offers incentive payments for mentorship, onboarding, participation in LTC‑specific events and projects, and an after‑hours care program.



Commitment to the care home and adherence to the five best‑practice expectations—including regular proactive visits, on‑site medication reviews, attendance at care conferences, involvement in 24/7 on‑call, and complete chart documentation—is required.

Practitioners working with LTCI homes collaborate with an interdisciplinary care team—physicians, nurse practitioners, nurses, allied health professionals, and care home leadership—to provide holistic, resident‑centered care. Current to LTC, you will receive community support to build skill and confidence.
