Description
Permanent part‑time Long Term Care (LTC) Practitioner position with St. Vincent’s Langara, a 196‑bed long term care home in Vancouver. The role expands a family practice by providing challenging, diverse, and impactful care to frail elderly residents.

Responsibilities

Assist with resident caseload for the LTC home, providing comprehensive, resident‑centered care.

Participate in interdisciplinary team meetings with physicians, nurse practitioners, nurses, allied health and leadership.

Meet the LTCI best‑practice expectations: regular proactive visits, on‑site medication reviews, attendance at care conferences, involvement in 24/7 on‑call, and completion of chart documentation.

Support mentorship and onboarding of recent practitioners within the LTCI program.





Engage in LTC‑specific events, projects, and after‑hours care program as part of the incentive structure.

Qualifications

Licensed medical practitioner with or willing to obtain Providence Health Care (PHC) privileges and credentials for the care home.

Experience or willingness to work within long‑term care settings.

Commitment to the formal LTCI program and its expectations.
