In this permanent role, you will assist with resident care, ensuring comprehensive services in the 196-bed facility. You'll participate in team meetings, adhere to LTCI best practices, and engage in mentorship and after-hours care initiatives.
Key Responsibilities:
• Provide resident-centered care for elderly residents
• Attend interdisciplinary team meetings for care coordination
• Complete proactive visits and medication reviews consistently
• Support current practitioners through mentorship initiatives
• Involve in after-hours programs and special projects
Requirements:
• Must possess medical licensure or secure PHC credentials
• Willingness or experience in long-term care settings
• Commitment to the standards of the LTCI program
