You will be responsible for gathering requirements and implementing digital tools aligned with the Western York Region Ontario Health Team's primary care strategy. By collaborating with various stakeholders, you will optimize workflows and develop tools aimed at reducing administrative burdens while improving patient care.

Key Responsibilities:
• Gather requirements for recent primary care digital initiatives
• Collaborate with diverse stakeholders to co-design tools
• Support chronic disease management through integrated pathways




• Provide troubleshooting and technical support to users
• Maintain system protocols and assist in upgrades

Requirements:
• Undergraduate degree in health informatics or related area
• 2–5 years of relevant clinical or EMR experience
• Certification in Epic preferred within 3 months
• Excellent communication and teamwork skills
• Familiarity with Microsoft Office and health IT systems
