About the Role
As a clinical pharmacist at Oak Valley Health, you will work with the inter‑professional team to provide clinical pharmacy services to Markham Stouffville Hospital (MSH), Uxbridge Hospital and Reactivation Care Centre (RCC) patients. MSH provides services to a wide range of patients in the following areas: Surgery, Medicine, ICU/CCU, Childbirth and Children, Psychiatry, Rehabilitation, Palliative, Complex Care, Oncology and Emergency. The Uxbridge Hospital provides services in Medicine, Complex Care, Emergency and Day Surgery. In this role, you will provide a range of drug distribution and clinical pharmacy services, participate in patient education, support teaching students and provide drug information to nursing and medical staff. You will work collaboratively with the pharmacists and pharmacy technicians as an integral member of our pharmacy team.

Responsibilities

Provide clinical pharmacy services to patients across MSH, Uxbridge Hospital and RCC.

Participate in patient education and support teaching students.

Provide drug information to nursing and medical staff.

Collaborate with pharmacists and pharmacy technicians.

Distribute medications and manage drug distribution processes.

Help identify, resolve and prevent drug‑related problems to optimize medication therapy and safety.

Qualifications

Graduated from an accredited university pharmacy program.

Licenced under Part A registration status with the Ontario College of Pharmacists.

Hospital pharmacy residency preferred.

Previous experience in a hospital setting with unit‑dose and IV admixture programs required.

Demonstrated experience identifying,



resolving and preventing drug‑related problems.

Demonstrated computer efficiency essential.

Excellent communication and interpersonal skills.

Evidence of professional development, continuing education and leadership experience preferred.

Demonstrated valuable attendance and performance records.

Commitment to a healthy and safe workplace, compliance with hospital policies, health and safety legislation and best practices, and completion of relevant mandatory education.

Work Schedule
The successful candidate will be available to work 8‑hour shifts days, evenings and weekends between Monday and Sunday, and on‑calls as required. Schedules may change due to operational needs.

Compensation
OPCOM09
Range: $51.29 - $67.47
