Join Pacific Community Resources Society as a Youth Case Management Worker in Chilliwack, BC. Support youth aged 13-18 facing substance use challenges through impactful case management services.

This full-time position involves being part of a multidisciplinary team dedicated to supporting youth in bed-based treatment. As a Case Management Worker, you will provide individualized care and facilitate collaboration with participants and their families. Essential skills include empathy, effective communication, and a holistic understanding of substance use and mental health.

Key Responsibilities:
• Provide assessment and intake for new participants
• Collaborate on individualized service planning
• Support participants’ self-care and emotional skills




• Guide families in accessing community resources
• Engage in team meetings for program evaluations

Requirements:
• 2-year diploma in Child and Youth Care (required)
• 3 years of experience in substance use treatment
• Solid case management and counseling skills
• Knowledge of community resources and harm reduction
• Ability to work both independently and as part of a team
