About the Role

The Rehabilitation Assistant supports residents in maintaining and improving mobility, strength, independence, and overall quality of life. Working closely with the interdisciplinary care team, the Rehabilitation Assistant helps deliver rehabilitation programs that support residents in achieving their individual goals and maintaining their highest possible level of independence.

Key Responsibilities:

- Assist residents with therapeutic exercises and rehabilitation programs
- Support safe mobility, transfers, and walking programs
- Encourage resident independence and participation in daily activities
- Monitor residents and report changes in their condition to the appropriate care team member
- Maintain accurate and timely documentation




- Collaborate with nurses and other members of the interdisciplinary care team
- Follow Infection Prevention and Control practices and workplace health and safety standards

Qualifications:

- Rehabilitation Assistant diploma or related health care education
- Experience working with older adults or in a long term care environment is considered an asset
- Strong communication, teamwork, and organizational skills
- Ability to work collaboratively as part of an interdisciplinary care team
