Position Summary
The position provides one-to-one case management, developmental support planning, mentorship, service coordination, and group facilitation to strengthen family capacity, improve developmental outcomes, increase youth stability, and reduce barriers to accessing community supports.
Key Responsibilities:
- Intensive Youth Case Management and Mentorship (Primary Role)
- Infant and Child Development Intensive Supports
- Infant and Child Development Group Facilitation
- Service Coordination and Community Collaboration
- Documentation, Evaluation, and Accountability
Required Qualifications:
- Diploma or degree in Social Work, Child and Youth Care, Early Childhood Education, Human Services, Psychology, Education, or a related field.
- Experience providing case management, mentorship, family support, or intensive individualized services.
- Experience working with children, youth, and families experiencing barriers or increased vulnerability.
- Knowledge of child development, youth development, and family support practices.
- Experience developing individualized service plans and coordinating community supports.
- Strong interpersonal, communication, and relationship-building skills.
- Ability to work independently while also collaborating effectively within a team environment.
- Valid driver’s license and reliable transportation.
Preferred Qualifications:
- Experience completing developmental screenings such as ASQ-3 and ASQ:SE-2.
- Knowledge of early intervention practices and developmental support planning.
- Experience working with youth navigating education, employment, housing, mental health, or social service systems.
- Training or experience in trauma-informed practice, Circle of Security, Brain Story Certification, resilience-based approaches, or culturally responsive practice.
- Experience working in rural communities.
Successful candidates must provide:
- Satisfactory Criminal Record Check with Vulnerable Sector Search.
- Satisfactory Intervention Record Check.
- Valid Standard First Aid and CPR Certification.
- Valid Class 5 Driver's License and valid registration.
- Reliable vehicle for travel throughout Grande Prairie and surrounding communities.
- Minimum $2,000,000 automobile liability insurance including business use.
Working Conditions
- Services may be delivered in office settings, participant homes, schools, community locations, partner agency spaces, and rural communities.
- Adaptable scheduling may be required to meet participant needs.
- Travel within Grande Prairie and surrounding rural communities may be required.
- Inclement weather may impact rural service delivery.
- The role involves working with individuals experiencing complex family, developmental, social, and emotional challenges
Pay: $27.00-$28.00 per hour
