POSITION TITLE: ABIN Supports for Community Living

REPORTS TO: ABIN Supervisor

Scope of the Position:

The Alberta Brain Injury Network Program Supports for Community Living worker is part of a larger provincial network providing service coordination of short term supports to individuals and families living with the impact of an acquired brain injury. Working from a trauma-informed perspective, the Supports for Community Living worker will promote a person-centered, rehabilitation approach to service delivery, respecting each client’s rights as an individual. Supports will be provided according to principles and philosophy of the Canadian Mental Health Association.

Key Responsibility: Support to Clients

● The Supports for Community Living worker will work with a Service Coordinator to identify specific, short-term goals for between 10-15 individuals, depending on hours.

● The Supports for Community Living worker will respect and protect the rights of service users, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, beliefs, religion, and culture; and their right to privacy and dignity.

● The Supports for Community Living worker will work with individuals in their own homes and in the community to improve their abilities and to develop capacity for independent living.

● The Supports for Community Living worker will assist individuals to set future goals and to identify achievable steps towards those goals to encourage success.

Key Responsibility: Reporting and Data Collection

● The Supports for Community Living worker will complete monthly reports, completion of service plan goal documentation,



extension letter requests and staying current with case note writing.

● The Supports for Community Living workers will assist with satisfaction surveys and other data tracking information required on a monthly/quarterly/annual basis and will report this information to the appropriate Service Coordinator as required.

● The Supports for Community Living worker will plan ahead regarding scheduling with clients to ensure timely delivery of service to each individual.

● Meet with the Supports for Community Living workers on a monthly basis on Teams for caseload conference and team building.

● The Supports for Community Living worker will complete other administrative duties as assigned (scanning, faxing, filing and copying) and must be proficient in Microsoft word and outlook, have some excel knowledge and be able to navigate through SharePoint.

● Must enjoy driving! Travelling to daily clients is a must

● Must have an active (Class 5 Driving License) with a clean driving record

● Company vehicles are supplied for travel

Qualifications

The ABIN Supports for Community Living worker would preferably have a diploma or degree in the human services field as well as a minimum of one years of experience that includes extensive knowledge in the area of brain injury and rehabilitation techniques.

Job Type: Full time

Pay: $22.50-$27.00 per hour
