The Chance
Are you passionate about helping seniors navigate their health journey with confidence and compassion? As a Health Navigator at Chartwell, you'll work closely with the Health & Wellness Manager to coordinate resident assessments, care plans, and wellness services. You'll collaborate with residents, families, physicians, and the care team to ensure every resident receives personalized, high-quality care in a welcoming retirement community.
What You'll Do
- Complete resident assessments and develop individualized care and service plans
- Coordinate resident admissions, care conferences, transfers, and discharges
- Provide nursing care, medication administration, and resident education as required
- Partner with physicians, families, and the care team to support changing resident needs
- Maintain accurate clinical documentation and monitor resident wellness outcomes
Support infection prevention, quality improvement, and regulatory compliance initiatives
What You'll Bring
- Registered Practical Nurse (RPN/LPN) or Registered Nurse (RN) in good standing with the provincial College of Nurses
- Current CPR/First Aid certification and medication administration knowledge
- Experience in retirement living, long-term care, assisted living, or a related healthcare setting
- Strong assessment, clinical, organizational, and communication skills
- Compassionate, collaborative, and resident-focused approach to care
- Experience working with electronic health records and Microsoft Office

Additional Requirements
- Ability to meet the physical demands of resident care
- Flexibility to respond to emergencies and participate in Manager-on-Duty or on-call rotations, where required
Clear Criminal Background Check, VSS and TB Test where required
