Position Title: Case Management Specialist
Location: Nova’s Ark – On-site
Employment Type: Full-Time
Wage: $22.00 – 25.00/ hourly - Commensurate with experience
Reports To: Operations Manager

Position Summary

The Case Management Specialist provides direct, person-centred support to youth with developmental disabilities participating in Nova's Ark programs who are not receiving services through the Ontario Autism Program (OAP). This role is responsible for developing and monitoring individualized goals, coordinating participant supports, collaborating with families and staff, and promoting each participant's independence, personal growth, and overall well-being while ensuring safety, dignity, and inclusion.

Essential Duties and Responsibilities

- Develop, implement, monitor, and review individualized goals for assigned participants.
- Ensure program activities are implemented in accordance with person-centred plans and individual goals.
- Facilitate participation in life skills, social skills, communication and recreation activities.
- Provide direct supervision and support to participants throughout the program day, as required.
- Assist participants with personal care, mobility, communication, and activities of daily living when necessary.
- Implement positive behaviour support strategies and trauma-informed approaches to address behavioural challenges.
- Support participants during behavioural or crisis situations, including the use of approved physical intervention techniques when required and appropriately trained.
- Ensure participant rights, dignity, choice, privacy, and confidentiality are maintained at all times.
- Communicate participant progress, achievements, concerns, and recommendations with families, guardians, and other authorized supports.
- Collaborate with Support Staff, Clinical Professionals (when applicable), and the Operations Manager to ensure consistent and effective service delivery.
- Participate in participant reviews, program planning, staff meetings, training, and professional development.




- Provide guidance and mentorship to Support Staff regarding participant goals and support strategies.
- Assist with program development and continuous quality improvement initiatives.
- Maintain qualified conduct while representing Nova's Ark in the community.
- Perform other duties as assigned to support program operations.

Qualifications

Education and Experience

- Diploma or degree in Developmental Services, Social Service Work, Child and Youth Care, Psychology, Behavioural Science, Human Services, or a related field.
- Minimum of one (1) year of experience supporting individuals with developmental disabilities, autism, or complex support needs.
- Experience with case management, goal planning, or individualized support planning is considered an asset.

Knowledge, Skills, and Abilities

- Knowledge of developmental disabilities, autism, and person-centred service delivery.
- Understanding of positive behaviour support, trauma-informed care, and crisis intervention principles.
- Ability to develop, implement, and evaluate individualized goals and support plans.
- Strong interpersonal, organizational, communication, and documentation skills.
- Ability to effectively collaborate with families, community professionals, and multidisciplinary teams.
- Ability to remain calm, professional, and solution-focused during challenging situations.
- Proficiency with basic computer applications and electronic documentation systems.
- Strong time management skills with the ability to manage multiple participant caseloads.

Physical and Environmental Requirements

- Ability to lift, transfer, position, or physically assist participants when required.
- Ability to remain physically active throughout the workday, including standing,



walking (up to 18,000 steps /day), bending, participating in community outings, and pushing wheelchairs.
- Ability to provide personal care assistance as required.
- Ability to respond appropriately during behavioural or medical emergencies.
- Ability to participate in outdoor activities with participants in all weather conditions.

Additional Requirements

- Current certification in CPI (Nonviolent Crisis Intervention®) is considered an asset. Employees who are not certified must successfully complete CPI training within three (3) months of their employment start date.
- Valid First Aid and CPR certification is an asset.
