Are you passionate about making an impact in your community? Do you have excellent organizational skills, attention to detail, and strong communication skills? We are seeking a highly motivated, detail-oriented Director of Care to support our team.
Key Responsibilities · Provide strong, visible clinical leadership to the multidisciplinary team (e.g. clinical managers, resource nurses, nurses, physicians, personal support workers)· Oversee and ensure the delivery of integrated, patient-centred care that meets the evolving needs of the community and adheres to best practices in palliative care.· Lead the development, implementation, and review of hospice clinical policies, procedures, and standards of care.· Drive quality improvement initiatives, including monitoring clinical key performance indicators (KPIs), analyzing clinical data, and reporting on outcomes to the executive director· Aligns programs and activities to the Strategic Plan· Manage and investigate patient complaints and clinical incidents, ensuring appropriate corrective action and a culture of learning.
Role Requirements
· Registered Nurse in positive standing with the College of Nurses
· Canadian National Certification in hospice palliative care (CHPCN) is an asset
· Minimum of 3 years of management or leadership experience in a health care setting preferred
· In-depth knowledge of hospice/palliative care principles, practices, and regulatory standards
· Proven experience in leading and managing diverse teams through change and fostering a culture of excellence.
· Exceptional leadership, communication, and interpersonal skills
· Highly developed analytical, problem-solving, and decision-making abilities
