We are seeking a dedicated RAI Coordinator to support resident care through expert coordination of InterRAI LTCF/MDS assessments, data accuracy, and clinical documentation. This role is ideal for an InterRAI certified RPN who is detail-oriented, collaborative, and committed to improving care outcomes in long-term care.

Responsibilities

- Promote a resident-centered approach by engaging residents, families, and staff to support individualized, dignified care.
- Create individualized care plans that aligns with resident wishes and report significant changes in behaviour, physical condition, or nutrition to the Director of Care/designate.
- Participate in multidisciplinary care conferences, committees, and planning meetings.
- Assist with resident admissions, transfers, discharges and end-of-life documentation.
- Advocate for residents and uphold the Resident Bill of Rights.

InterRAI LTCF Coordination

- Coordinate the scheduling and completion of all InterRAI LTCF assessments within mandated timelines.
- Ensure accuracy of coding, completion of CAPs, care plan updates and compliance with CIHI and MOHLTC standards.
- Provide education, coaching and software training (PointClickCare, Relias Mastering interRai) to nursing and interdisciplinary team members.
- Support staff with coding questions, validate assessments and assist with resolving submission errors.
- Integrate InterRAI assessments with clinical tools and streamline documentation processes within PCC.

Quality Improvement, Data Management & Audits

- Conduct monthly and quarterly audits of POC and interRAI assessments; analyze results and collaborate with leadership on corrective actions.




- Generate and interpret RAI-MDS/interRai outputs, quality indicators (QIs) and other reports to support clinical and operational decision-making.
- Oversee data submission processes and act as liaison with Health Zones, AH&W;, CIHI and PCC.
- Develop and refine processes that improve data accuracy, workflow efficiency and interRai utilization.
- Support Nursing Restorative assessments, evaluations and discharges in collaboration with RNs, RPNs and Restorative staff.

Compliance, Safety & Best Practices

- Support quality and risk management initiatives to maintain a secure and secure environment for residents and staff.
- Ensure compliance with the Fixing Long-Term Care Homes Act (2021), O. Reg. 246/10 and relevant organizational policies.
- Implement and promote RNAO best practices throughout nursing care.

Qualifications

- Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
- InterRAI LTCF CIHI Essentials (Modules 1–3) certificate from an accredited provider.
- Experience in long-term care; InterRAI/MDS experience is an asset.
- Medication administration certificate from a recognized college or university
- Proficiency in PointClickCare, Microsoft Office and computerized documentation systems.
- Strong communication, leadership and relationship-building skills.
- Ability to analyze clinical data and understand RUG levels, QIs and case mix indicators.
- Satisfactory Vulnerable Sector Check (within 6 months).
- Two-step TB test or chest X-ray ( must be completed within 14 days of hire).

Job Type: Part-time

Pay: $30.00 per hour
