Responsibilities

- Conduct outreach activities to educate community members about available health services and resources
- Collect and document data related to community health needs, service utilization, and program outcomes
- Assist individuals with disabilities by providing tailored support and ensuring they access appropriate healthcare options
- Deliver engaging teaching sessions on health topics in various community settings
- Serve as a public speaker at local events to raise awareness about health issues and available programs




- Collaborate with healthcare teams to coordinate care and follow-up for underserved populations
- Maintain accurate records of interactions, data collection efforts, and service delivery activities

Experience

- Prior experience working directly with community members or in a public health setting is highly preferred
- Demonstrated ability to work effectively with individuals with disabilities or special needs
- Strong teaching skills with the ability to communicate complex information clearly and confidently
- Experience in public speaking or facilitating group discussions is advantageous
- Proven skills in data collection, documentation, and maintaining detailed records
- A proactive attitude with excellent interpersonal skills and cultural sensitivity
