Riverdale Immigrant Women’s Centre is hiring on behalf of an employer partner for a full-time Case Manager chance in Toronto.
The Case Manager will provide case management and support services to older adults with complex mental health, addiction, and physical health needs, particularly individuals receiving services within the long-term care system. The role focuses on helping clients establish safe, realistic goals while coordinating appropriate health, social, and community supports.
Duties:
- Conduct client intakes and needs assessments
- Support older adults with complex mental health, addiction, and physical health needs
- Develop, implement, and monitor individualized care plans with short- and long-term objectives
- Provide assessment, intensive case management, long-term planning, and health and wellness support
- Respond to crisis calls from clients, family members, health-care providers, and community agencies
- Provide consultation to long-term care homes supporting clients with active addiction concerns
- Support clients using recovery-oriented and harm-reduction approaches
- Coordinate and link clients with appropriate health, social, and community services
- Collaborate with the City of Toronto, health-care providers, community organizations, and other service systems
- Work closely with transitional care, hospitals, emergency departments, rehabilitation centres, and other professionals to identify service gaps and appropriate solutions
Experience & Qualifications:
- University degree and/or college diploma in Social Services or a related field
- Minimum three years of related experience providing direct services to adults with responsible mental illness
- Experience supporting individuals with mental health and substance-use concerns using recovery and harm-reduction approaches
- Demonstrated case management and referral skills
- Ability to effectively connect clients with appropriate supportive services
- Excellent verbal and written communication skills
- Ability to work independently and collaboratively with internal and external stakeholders
- Strong computer skills, including Microsoft Office, client information systems, and web-based applications
- Fluency in a second language is a strong asset
Requirements:
- Valid driver's licence and access to a personal vehicle
- Criminal background check
- Ability to work flexible hours, including evenings and weekends as required
- Must be legally eligible to work in Canada
