Pay: $49,913.40–$56,976.26 per year

Riverdale Immigrant Women’s Centre is hiring on behalf of an employer partner for a full-time Case Worker opportunity in Toronto. The Case Worker will provide ongoing case management, service coordination, and care planning support to clients, with a particular focus on seniors and individuals experiencing age-related mental and/or physical health concerns.
The successful candidate will work closely with clients, caregivers, health-care providers, community agencies, landlords, and other professionals to identify needs, coordinate appropriate supports, and promote client well-being and independence.
Duties:
- Conduct intake interviews and client needs assessments
- Develop, implement, monitor, and revise individualized care plans
- Provide ongoing case management and service coordination
- Support clients through advocacy, coaching, risk assessment, and crisis prevention/intervention
- Connect clients to primary care, financial assistance, assistive devices, food programs, transportation, recreational programs, and other community resources
- Coordinate services and communication between clients, caregivers, health-care providers, landlords, and community agencies
- Work collaboratively with seniors services, hospitals,
rehabilitation centres, EMS, transitional care providers, and other professionals
- Maintain accurate client records, charts, and workload documentation
- Develop case plans using the Ontario Common Assessment of Need (OCAN) or Inter-RAI Community Health Assessment tools
Experience & Qualifications:
- College diploma or university degree in Social Services or a related field; BSW or MSW considered an asset
- Minimum three years of related experience in seniors case management and care coordination
- Experience working with clients with age-related mental and/or physical health concerns
- Strong knowledge of seniors' issues, community services, and available supports
- Demonstrated case management, assessment, advocacy, and referral skills
- Excellent verbal and written communication skills
- Ability to work independently and collaboratively as part of a multidisciplinary team
- Strong computer skills, including Microsoft Office, client information systems, and web-based applications
- Fluency in a second language is a strong asset
Requirements:
- Valid driver's licence and access to a personal vehicle
- Criminal background check
- Ability to work flexible hours, including evenings, weekends, and statutory holidays as required
- Must be legally eligible to work in Canada
