Responsibilities

- Deliver direct patient care by administering medications, performing wound care, and monitoring vital signs with precision and compassion
- Assist with patient assessments, documenting medical histories and current conditions accurately using electronic health records such as Epic or Cerner
- Support patients with activities of daily living, including mobility assistance and personal hygiene in various settings like nursing homes or home care visits
- Collaborate with interdisciplinary teams to develop and implement individualized care plans focused on pain management, dementia care, or senior support
- Conduct phlebotomy procedures and laboratory specimen collection adhering to safety protocols




- Educate patients and their families about health conditions, medication regimens, and self-care strategies to promote wellness and independence
- Maintain compliance with medical terminology standards and ensure all documentation aligns with regulatory requirements

Requirements

- Valid Licensed Practical Nurse (LPN) license in good standing
- Proven experience in nursing home settings or hospital environments; acute care experience highly preferred
- Familiarity with dementia care, hospice support, pediatrics, or assisted living environments is advantageous
- Strong knowledge of medication administration protocols and medical terminology
- Experience working with electronic health record systems such as Epic or Cerner is preferred
- Ability to perform phlebotomy and laboratory procedures accurately and safely
- Excellent patient care skills combined with effective communication abilities
- Prior experience in home care or case management is a plus
- Compassionate attitude with a proactive approach to patient safety and comfort
