They offer in-home support to families facing various challenges, including but not limited to parenting difficulties, social isolation, financial strain, mental health issues and promote overall well-being.
Duties and Responsibilities
- Conduct comprehensive assessments of families' needs and strengths.
- Develop individualized support plans in collaboration with families, setting achievable goals and objectives.
- Provide practical assistance and emotional support to families, parenting skills training, household management, budgeting, and accessing community resources.
- Offer guidance and encouragement to enhance family communication, problem-solving, and conflict resolution skills.
- Advocate on behalf of families to access relevant services and resources within the community.
- Maintain accurate and confidential documentation of all interactions and interventions with families.
- Collaborate effectively with other professionals and agencies involved in the family's care, including social workers, healthcare providers, and educators.
- Participate in ongoing career development activities to enhance knowledge and skills related to family support practices.
- Undertake various duties as assigned to support the overall objectives of the organization and meet the evolving needs of the community.
Requirements:
- Certificate or diploma in a Human Services or related field; alternatively, a combination of relevant education and experience
- Previous experience working with diverse families facing various challenges.
